Package will be formatted to follow the "property ownership list and radius map" requirements in the submittal package under the filing requirements.
City of Upland Filing Requirements. Property ownership list and radius map as follows:
Two (2) sets of typed, gummed labels listed the names, addresses, and the Tax Assessor's Parcel Number of all property owners and residents (when different than the property owner) within 300 ft. of the exterior boundaries of the subject property.
The list shall be obtained from the latest Equalized Assessment Rolls issued by the San Bernardino County Tax Assessor

I hereby certify that the attached property owner's list is a true and complete list of owners and residents within a 300 ft. radius of the subject property, based on the latest equalized county tax assessor's rolls, and that the information is true and correct to the best of my knowledge.
The mailing list also conforms to the format requirements of the City's sample mailing list.
I understand that submittal of incorrect or incomplete mailing labels is grounds for continuation and/or re-notice of the public heating for my application, all at my sole cost and expense.
