Bona Fide Restaurant, select 300' Distance
This will include the standard 300' Notification list and a 1000' map of schools/parks, etc & a 300' list/map of the name and address of all establishments that sell alcohol for on-site and off-site consumption within a 300-foot radius. 

Non Bona Fide Restaurant, select 500' Distance.
This will include the 500' Notification list + OCCUPANTS and a 1000' map of schools/parks, etc & a 300'  list/map of the name and address of all establishments that sell alcohol for on-site and off-site consumption within a 300-foot radius.

Full Description from the city package (City of Montebello)

ALL alcohol related Conditional Use Permits must provide the following information:

1) Provide a map that illustrates the location of all schools (public or private), parks, playgrounds, recreational facility, or other similar uses that are within 1,000 feet of the project site.

2) List by name and address all establishments that sell alcohol for on-site and off-site consumption within a 300-foot radius of the project site. Use additional sheets if necessary. Also, provide a map detailing the location of the following uses with respect to the subject site. Address Business Name On or Off-Site Sales

Radius Map Map identifying all properties located within the required radius from exterior boundaries of project site. Each affected lot must be numbered (with project site as number 1). The number corresponds to addresses included in the mailing list. Radius varies: 300 feet standard, 500 feet for commercial/healthcare facilities, 1000 feet for schools.

Mailing List Complete list of property owners within the required radius from project site. If radius line touches any portion of lot, it must be included. List includes property owner's name, mailing address, and Assessor Parcel Number. Obtained from latest Los Angeles County Assessor records (East, South, or West District Offices).

Self-Adhesive Mailing Labels Self-adhesive mailing labels on standard 8½" x 11" sheets with names and mailing addresses from mailing list. Please provide additional mailing list and corresponding reference numbers for applicant, owner of subject site, and/or architect. Two copies of radius map, mailing list, and labels required.

Certified Affidavit Affidavit at end of mailing list certifying accuracy of radius map, property owner information, and all other public notification requirements. Signed under penalty of perjury verifying information was obtained from latest LA County Assessor records. Express Mapping completes and certifies for you.
