Packaged will be formatted to follow the "Public Notice Radius Map Guidelines" requirements. 

300' Radius Map:
Applicants must submit an original and one copy of a radius map showing all properties within a 300-foot radius of the subject property. Each parcel on the map must be labeled with a reference number that corresponds to the ownership list.

Property Owners List:
A complete ownership list must be prepared from the latest Assessor’s records. The list must include the names and mailing addresses of all property owners within 300 feet of the project site.

Mailing Labels:
Two sets of addressed, self-adhesive gummed mailing labels must be provided. Labels must include the Assessor’s parcel number, owner’s name, mailing address, city, state, and ZIP code. Labels must be prepared using Avery 5162, 5351, or 5375 formats. Express Mapping uses Avery 5160 Labels, 3 across, 10 down.

Affidavit:
A signed and dated affidavit is required, verifying that the ownership list and radius map are accurate and based on the most recent Assessor’s roll. The affidavit must be completed by the applicant and submitted with the application package. Express Mapping will complete this affidavit on your behalf.
