100'/300'/500'-foot vicinity map:
Applicants must submit a vicinity map showing all properties located within 100 feet of the subject property, even if they fall outside city limits. The map must be drawn from the exterior boundaries of the property and prepared using Los Angeles County or Ventura County Tax Assessor’s maps. Each lot must be consecutively numbered and match the property owner list.
100'/300'/500' Property owners list:
A complete property owners list must be typed and submitted along with two sets of gummed address labels. Handwritten lists are not accepted. For each lot within the 100'/300'/500'-foot radius, the list must include the legal description, the sequential number, the owner’s name, and the mailing address. Property information must be sourced from the latest available county assessor’s roll.
Certified affidavit:
Applicants must also provide a signed and notarized affidavit certifying that the attached property owners list includes all names and addresses of persons whose property is assessed within 100'/300'/500' feet of the subject site, based on the latest available assessment roll.
