Packaged will be formatted to follow the submittal requirements from the Application.

A map indicating all properties within 300' of the exterior boundaries of the subject site, and a correlated property owners list providing the property owners’ names and addresses for these properties (See Sample “C”). This certified list must be prepared from the last equalized assessment roll of the County of Los Angeles Tax Assessor’s Office.

300'Radius Map:
Applicants must provide a map indicating all properties located within 300 feet of the subject site’s exterior boundaries. The map must be correlated with a certified property owners list prepared from the most recent Los Angeles County Tax Assessor’s roll. Each parcel on the map must be numbered to correspond with the property owners list.

Property Owners List:
A complete property owners list is required, containing the names and mailing addresses of all property owners within the 300-foot radius. The list must be certified as accurate and prepared from the latest equalized assessment roll.

Gummed Mailing Labels:
Two complete sets of gummed address labels must be submitted. These labels must reflect the names and addresses listed in the 300-foot property owners list and correspond directly to the numbered parcels on the radius map.
