500' Base Map (Scale 1" = 100')
Applicants must submit a base map showing the subject property and all adjoining parcels located within 500 feet. The map must be drawn to a scale of 1 inch equals 100 feet. Each parcel within the 500-foot radius must be numbered to match the property owners listed on the mailing labels.
Gummed Mailing Labels (Property Owners List)
Two complete sets of gummed mailing labels must be prepared. Each label must include the names and addresses of all property owners within 500 feet of the subject property. Labels must be typed, numbered to correspond with the parcels on the base map, and formatted on 8.5 by 11 inch sheets in three columns of ten or eleven rows. Only Xerox matrix or equivalent formats will be accepted.
Property Owner’s List Preparer’s Affidavit (Notarized)
The preparer of the property owners list must complete and sign a notarized affidavit. This affidavit certifies that the names and addresses on the list are taken from the most recent Los Angeles County Assessor’s roll and include all properties within 500 feet of the subject property.
