Property Owners Map and Mailing Labels (300-foot Radius):
Applicants in the City of Rosemead seeking a Conditional Use Permit are required to submit a property owner’s radius map and mailing labels. The radius map must identify all parcels within or partially within a 300-foot radius of the subject property boundaries. Each parcel will be numbered (#1, #2, #3, etc) and correspond to a property owner’s list.

Mailing Labels and Ownership List:
Three complete sets of gummed mailing labels must be provided, containing the assessor’s parcel number, property owner’s name, and mailing address for each property within the 300-foot radius. The property ownership list must be based on the most current Los Angeles County Assessor’s roll to ensure accuracy. One list of assessor's parcel number, property owner, and property owners mailing list for each parcel numbered (#1, #2, #3, etc)

Certification Requirement:
A notarized affidavit must be included with the submission, certifying that the property owners’ list and mailing labels have been prepared in accordance with the City of Rosemead’s requirements. This ensures compliance with public noticing obligations prior to any hearings or approvals. One copy of this form with the affidavit shown below signed and notarized by the preparer (Express Mapping) of the three items (Map, List, Labels) listed above.
