Full Description from the city package (City of Pasadena)

Packaged will be formatted to follow the "Notification Package" requirements from the Application

Radius Map: A scaled radius map must be prepared showing all properties located within 300 or 500 feet of the project site, depending on the type of application. Each property within the radius must be numbered to correspond with the ownership list.

Ownership List: Applicants must provide a typed ownership list of all properties within the required radius. The list must include parcel numbers, property owner names, and mailing addresses obtained from the latest records of the Los Angeles County Assessor.

Mail Merge Excel Spreadsheet: Applicants must provide an Excel file with the ownership list for city mail merge use. The spreadsheet must include radius map reference number, property owner’s full name, street address, city, state, and ZIP code. All information must be typed in CAPS ONLY. The file must be saved on a USB flash drive and submitted as part of the notification packet.

Affidavit: A notarized affidavit must be signed and dated, certifying that the ownership list and radius map are accurate and based on official assessor records.
