Packaged will be formatted to follow the "Mailing Information" requirements from the Application Package Requirements.

Radius Map
Applicants must submit a radius map showing all properties within 300 feet of the subject property. The map must include parcel boundaries, highways, streets, alleys, and all lots or parcels of land within the radius. Four physical copies or a digital file are acceptable.

Mailing Labels
A certified list of property owners and tenants must be prepared using the most recent Los Angeles County Assessor’s roll. The list must include names and addresses of all persons whose property falls within 300 feet of the subject property. Plain mailing labels must be provided with all required addresses.

Preparation Affidavit
An affidavit with a notarized signature from the list preparer is required. The affidavit certifies that the radius map and mailing labels are accurate and comply with the city’s requirements.
