Radius Map
The radius map is required to identify properties that must receive written notice of a public hearing. A 300-foot radius is drawn from the perimeter of the subject property. All owners of properties within or touching this boundary will be notified. The ownership map must include parcel boundaries and be accurate to avoid delays or re-noticing.

Property Ownership List and Mailing Labels
A property ownership list and mailing labels must be prepared using the latest available Los Angeles County Tax Assessor’s roll. The list must include the Assessor’s Parcel Number and the property owner’s name and mailing address. Two sets of typed or printed self-adhesive labels (1” x 2 5/8”) are required. Labels must also include the applicant and any other parties who need written notice of the hearing.

Certified Property Owner’s List Affidavit
An affidavit must be signed certifying that the ownership list contains the correct names and addresses of all persons to whom property is assessed. The certification must confirm that the list was created from the most recent County Assessor’s roll and covers a distance of 300 feet from the subject property boundaries. The affidavit must be notarized before submission.
