Packaged will be formatted to follow the "Notification Package" requirements from the Application

500′ Radius Map:
The radius map must depict all parcels located within a 500-foot radius from the exterior boundaries of the subject property. Each parcel must be numbered to correspond with the ownership list. The map must clearly identify the subject property and show the surrounding properties affected by the proposed project.

Ownership List Requirements:
The ownership list must include the names and mailing addresses of all property owners and occupants within a 500-foot radius as shown on the most recent Los Angeles County Assessor’s Roll. The list must be prepared by a qualified preparer and notarized to certify accuracy. It must also include the applicant’s name and mailing address, as well as the City Planning Division and the applicant noted on the mailing list.

Mailing Labels:
Two (2) sets of self-adhesive mailing labels, plus one photographic copy of the list, are required. Labels must include the Assessor’s Parcel Number (APN), property owner’s name, street address, city, state, and ZIP code. The labels must be typed or printed clearly and correspond exactly with the ownership list and radius map. Labels must not be transparent and must fit 30 labels per sheet with a maximum width of 3 inches.

Map Numbering:
Each parcel shown on the radius map must be numbered consecutively to match the ownership list and mailing labels. These numbers allow the City to verify that all property owners within the notification boundary have been properly identified.

Certified Property Owners List Affidavit:
A notarized affidavit must be completed by the qualified preparer certifying that the ownership list and mailing labels are true and correct. This affidavit confirms that all property owners within the 500-foot radius have been accurately included based on the latest County Assessor’s information.
