    Full Description from the city package (City of San Fernando)

    Packaged will be formatted to follow the "Notification Package" requirements from the Application

    500′ Radius Map:
    The radius map shall show all parcels within a 500-foot distance from the exterior boundaries of the subject property. Each parcel must be consecutively numbered to correspond with the ownership list. The map must clearly indicate the subject property by graphic means and identify all surrounding parcels included within the 500-foot notification area.

    Ownership List Requirements:
    The ownership list must contain the names and mailing addresses of all property owners within the 500-foot radius, as they appear on the latest Los Angeles County Assessor’s Roll. The list must also include the applicant’s name and mailing address, marked with “XX,” along with any representatives or consultants who are to be notified by certified mail.

    Mailing Labels:
    Two sets of gummed mailing labels and one copy are required. Each label must include the property owner’s name, mailing address, city, state, ZIP code, and Assessor’s Parcel Number (APN). The first label must show the applicant’s name and address and include the “XX” marking. All labels must be keyed to the parcel numbers on the map.

    Map Numbering:
    Each property on the radius map must be numbered consecutively, corresponding to the list of property owners. These numbers must appear clearly on both the map and the ownership list to ensure accurate notification tracking.

    Certified Property Owners List Affidavit:
    The preparer of the 500′ radius map and mailing labels must sign a notarized affidavit certifying that the ownership list is true and correct according to the most recent assessment roll of Los Angeles County.
