The map must identify all parcels by Assessor’s Parcel Number (APN) and include all properties located within the applicable radius. For General Plan Amendments that increase residential density, the radius requirement is 2,000 feet.

Labels must also include addresses for public agencies within the boundary. Labels should be prepared from the most recent Los Angeles County Assessor’s roll or updated records. The labels must be on Avery 5160/5260 or equivalent sheets, sorted by APN in ascending order, and duplicate owners must be identified.
Notarized Certification Form:
A completed and notarized certification form must be provided, signed by the preparer of the radius map and mailing labels. This certification confirms that the information is true and correct to the best of the preparer’s knowledge.
