Packaged will be formatted to follow the "Notification Package" requirements from the Application

500′ Radius Map:
Applicants must submit a map drawn to scale showing all schools, parks, residences, churches, and other alcohol outlets located within a 500-foot radius of the project site. The map must clearly outline the property boundaries and identify each alcohol-related establishment or sensitive use within the radius.

Ownership List Requirements:
A certified list of property owners and tenants located within the required notification radius must be provided. The list must include complete names, addresses, and ZIP codes, as they appear on the most recent Los Angeles County Assessor’s Roll. All property owners and tenants within the radius must be listed.

Mailing Labels:
Two (2) sets of self-adhesive mailing labels must be provided, showing the names and mailing addresses of all property owners and tenants within the required 300-foot or 500-foot radius. Labels must be typed on 8½″ × 11″ sheets, arranged in three columns with eleven labels each. Labels must be legible and correspond exactly to the certified property owners list.

Certified Property Owners List Affidavit:
The certified property owners list must include a signed and dated affidavit confirming that the information is accurate and derived from the most recent assessor data. The affidavit certifies that all parcels and occupants within the specified radius have been properly included.
