Full Description from the city package (City of Monterey Park)

Packaged will be formatted to follow the "Notification Package" requirements from the Application

Radius Map Two copies of professionally prepared 300 foot radius map extending from subject property. Map must include Assessor's Parcel Number (APN) of all properties involved. 8.5" x 11" format is acceptable if legible. Shows all properties that fall at least partially within the 300 foot radius.

Mailing Labels Complete set of adhesive mailing labels, 30 per page, for all property owners whose properties fall at least partially within 300 radius of subject property. Labels must include name, address, city, state, zip code, and APN. One photocopy of labels stapled for submission.

Resident Labels For projects on residential property, labels must also be provided for current residents within the 300 foot radius. Ensures complete notification coverage for all residential developments requiring public hearings.

Label Preparer's Affidavit Affidavit certifying that the attached list represents addresses of all property owners located within 300 feet of the exterior boundaries of the names and addresses as obtained from latest Los Angeles County Assessor Rolls. Must be signed and notarized by label preparer.
