Full Description from the city package (City of Gardena). Two (2) sets of Radius Map and Property Owner/Occupant List on mailing labels are required for Conditional Use Permit, Mergers/Lot Line Adjustment, Parcel Map, Site Plan Review, Tract Map, Variance, Zone Change and Adminstrative Adjustments.

Packaged will be formatted to follow the "Radius Map and Property Owner/Occupant List Instructions" requirements from the Community Development Department application. 
Two (2) sets of Radius Map and Property Owner/Occupant List on mailing labels

300' Radius Map:
Applicants must provide a radius map showing the project site and all properties within 300 feet. The map must include street names, addresses, zoning, and lot dimensions. Two sets of original address labels and one photocopy set must be submitted. Labels must list the name and address of each property owner, occupant, condominium, apartment unit, or business located within the 300-foot radius, along with the applicant and representatives. Radius map (8½” x 11” size) showing the project site and all lots within a radius of 300 feet from the project site, including the street names, addresses, zoning, and lot dimensions

Original Address Labels:
Original address labels (two sets, 8½” x 11” size). Labels shall include the name and address (including condominium/apartment/unit/suite number or letter) of all property owners, occupants/tenants, and businesses located within 300 feet of the project site, as well as the name and address of the applicant(s), his/her representative, and any other person(s) the applicant wishes to be notified of the public hearing.

Notarized Certified Affidavit:
A certified affidavit is required confirming that the attached list contains the names and addresses of all property owners, multi-family units, and businesses within the 300-foot radius as shown in the latest Los Angeles County assessment roll. The affidavit must be signed, dated, and notarize of property owners/occupants/tenants/businesses list as specified above.
