Packaged will be formatted to follow the requirements from the CUP or Variance Application.

Surrounding Property Ownership Map
Submit one map at a scale of 1 inch = 100 feet showing all properties included in the request within the required radius of the subject site. The radius depends on the project: 500 feet for sites under 3 acres; 700 feet for sites 3 acres and above or when an environmental impact report is required; 1,000 feet for hillside areas when warranted. Parcels must be shown and numbered.

Property Ownership List and Mailing Labels
Prepare a completed ownership list from the latest Los Angeles County Assessor’s roll. Provide two sets of self-adhesive address labels identifying each surrounding property owner’s name and mailing address. Labels must be consecutively numbered and cross-referenced to the parcels on the ownership map and list.

Certified Property Owner’s List Affidavit
Provide an affidavit certifying that the ownership list is correct and based on the latest assessment roll, and that it covers owners within the applicable 500-, 700-, or 1,000-foot radius of the subject property. The affidavit must be signed under penalty of perjury.

Provide an ownership radius map, list and labels as follows:
a. A scaled map showing all parcels located within or intersected by a
1,000-foot radius of the subject property’s boundaries. The map shall
identify the Assessor’s Parcel Number (APN) for each such parcel.
b. An ownership list for all parcels within or intersected by the 1,000-
foot radius boundary, certified by affidavit to have been obtained from
the latest Assessor’s Rolls. Each entry on the list shall include the
APN, the property owner’s name, the corresponding parcel’s street
address, and the property owners mailing address if different from
the parcel address.
c. Two separate sets of self-adhesive mailing labels, with each label
containing the APN, the property owner’s name, and the property
owner’s mailing address. If a property owner’s mailing address differs
from the associated parcel address, then an additional label shall be
created that is addressed to “current resident” and include the
mailing address for the applicable parcel. All labels shall be crossreferenced
